Documentation Guidelines

Students who request to be considered for reasonable accommodations must submit current written documented verification from a licensed physician, psychologist, or other qualified professional. A school plan such as an IEP or 504 Plan is not sufficient documentation, but may be helpful in determining a student’s eligibility status.

It is the responsibility of the student to obtain his or her documentation and to present a copy to the Disability Management System--Student Disability Access Office. Correspondence regarding adequacy of the documentation will be sent to the student. It is also the student’s responsibility to obtain additional information or testing when requested to do so.

To be considered for reasonable accommodations, you must apply directly to the Student Disability Access Office. The Watts School of Nursing Office of Admissions does not forward documentation or a student’s notification of a disability to our office.

Please be aware that Duke University/Watts School of Nursing reserves the right to determine if a condition or diagnosis rises to the level of a disability under the ADA and what constitutes reasonable accommodations according to the documented nature of the disability as well as the fundamental nature of the program.
